The third session of our 2021 Webinar Series will be held on 18 August 2021, 2.30-3.30 pm (Singapore time)! It will be a live 1 hour session with a Q&A segment at the end with the speaker.

We are honoured to have Dr Chong Poh Heng (Singapore) speaking on the topic of Sending the dying patient home to die in a Terminal Discharge – Indications, Process, Outcomes! You will be able to ask Dr Chong any questions you may have at the end of the session.

Dr Chong is Medical Director of HCA Hospice Care, a community specialist palliative care service in Singapore. As the largest hospice provider looking after more than 50% of home care needs locally, it supports patients at home and offers palliative day care. It also runs the only paediatric palliative home care service called Star PALS. Other than imagining new ways to enhance client support, Dr Chong’s passion and interests lies in advocacy, innovation and research.

We believe that you will be able to get the latest updates and enjoy the meaningful discussions during this webinar session.
