Children with medical complexity (CMC) are a medically fragile population that are defined as individuals with high family identified needs, complex chronic disease, functional disability and high health care utilization. Medical advancements have led to a growing cohort of CMC and the medical home has emerged as a healthcare model to serve the unique needs of CMC and their families. Concurrent with the rise in prevalence of CMC, the field of specialty paediatric palliative care (PPC) has grown in expertise to improve quality of life, reduce distressing symptoms and provide end of life care for children with life-limiting illnesses. With a case study, this session will explore how principles of palliative care and the CMC medical home are complementary, ways PPC specialists can best support CMC as “medical neighbours” and aspects of PPC that all clinicians caring for CMC can learn to provide.
Dr Grace Ng is an associate consultant with Star PALS, a paediatric palliative home care service under the organization HCA Hospice Care, Singapore. She completed her paediatric residency in National University Hospital Singapore and went on to pursue a one-year fellowship in paediatric complex care at Montreal Children’s Hospital, Canada, where she was awarded the David McCutcheon Fellowship in Paediatrics. Her academic interests are in the intersection of paediatric complex and palliative care, and in advocacy for this group of patients.
